Mail-In Rebate Terms & Conditions: |
| Rebates are dependent on the rate plan chosen and the following criteria described as follows: |

- Customer is only eligible for a rebate on every device that the wireless Carrier has approved their credit for that
does not require a deposit or spending limit.
- All rebates require an activation of a voice plan $39.99 or higher and data devices require an activation of a
$39.99 or higher voice plan and an unlimited data plan to be eligible.
- The amount of the rebate cannot exceed the cost of the device on customer's first bill or equipment
invoice.
- A rebate form must be submitted for each device separately and we suggest making copies for your records. You must
submit the completed and signed rebate form with a copy of your first invoice clearly showing the amount you were charged for your equipment and your first bill (may be the same document
in some instances).
- Mail copies to the address on the rebate form. All rebates will be processed on the 30th day from the original
activation date. Please allow 10 to 12 weeks to receive your rebate check following the 30th day. Note:
- *Your rebate check will never exceed the actual cost of your equipment listed on your "Equipment Invoice" or
"Wireless Carrier Bill".
- Mail-In Rebates are only available for NEW MAIN Line Activations and not elegible for Migrations, Upgrades, Contract
Renewals and Family Shared extra lines 2-5.
